Choose an appointment time below that works best for you. Don’t see a time that fits your schedule? Give our office a call at 920-287-73019! We reserve a few additional appointment times that aren’t available through online booking, and we’d love to help find a time that works for you!

Your First Visit

  • New Patient Appointment: $110

  • X-rays (if recommended by the doctors): $65 per image, with a maximum of 3 images.

What Happens After Booking?

Once you’ve scheduled your appointment, you’ll receive your new patient paperwork electronically. Please complete these forms before your visit so our doctors can review your information and be fully prepared for you!

One of our friendly team members will also give you a call to confirm your appointment, answer any questions, and collect a deposit to secure your reserved appointment time.
